Application

We have two forms of the application available online to better serve the various internet browsers and software that you may be utilizing.

1. Standard browser format, compatible with most browsers. For best printing results please maximize window. Reception Application - Standard Version
   
2. Adobe Format. This format will produce the best results when printing but you must have Adobe Acrobat Reader installed on your computer, see below for a free download. We do not recommend this format if you have a slow connection speed to the internet.
